Book your appointment online or call our office. We offer in-person visits in Mount Juliet and convenient telemedicine options.

Complete Paperwork

Arrive 15 minutes early to complete new patient forms, or request them ahead of time to fill out at home for a smoother check-in.

Initial Evaluation

Your comprehensive assessment typically takes 60-90 minutes. We take time to listen to your health history, concerns, and wellness goals.

Treatment Plan

Together, we'll create a personalized care plan that respects your values and preferences, supporting your unique health journey.

What to Bring

Photo ID

Valid government-issued identification such as a driver's license or passport.

Insurance Card

Both sides of your current insurance card, including any secondary coverage information.

Medication List

Complete list of all current medications, supplements, and vitamins including dosages and frequency.

Prior Medical Records

Recent test results, imaging reports, or records from previous providers if available.

Emergency Contact

Name and phone number of a trusted person we can contact in case of emergency.

List of Questions

Write down any health concerns, symptoms, or questions you'd like to discuss during your visit.

Patient Forms

To streamline your first appointment, patient forms are available at our office or can be completed when you arrive. We recommend arriving 15 minutes early to allow time for paperwork.

Telemedicine Visits

Access quality care from the comfort of your home through secure virtual appointments. Telemedicine is available for many types of visits including follow-ups, medication management, and minor illness consultations.

Office Policies

Cancellation Policy

We understand that schedules change. Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient who may need care. Late cancellations or no-shows may be subject to a fee.

Late Arrivals

Please arrive 15 minutes before your scheduled appointment time, especially for your first visit. If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure quality care for all patients and stay on schedule.

No-Show Policy

Missed appointments without notice prevent other patients from receiving timely care. Repeated no-shows may result in dismissal from the practice. We value your time and ask that you respect ours and that of other patients.

Payment & Insurance

Payment is due at the time of service. We are a cash-pay, out-of-network practice and do not bill insurance directly, but we can provide a superbill you may submit to your insurer for possible out-of-network reimbursement. Our per-visit pricing is transparent and posted on our Pricing page, so you always know your cost up front.
